We ask that all new patients to the Cottage Surgery please call us to book their first appointment. New patient appointments are 30–40-minute long and cannot be booked online.
For new patients with specialty bookings such as Childhood vaccinations, Aviation and Dive Medicals, Iron and other infusions these may require a longer appt with the GP and/or the Practice Nurse. Please call on 02 9948 3768

We ask you to please submit your new patient form after you have booked your appointment. Our new patient form captures important demographic information to ensure that your health record is up to date, and in order for us to provide you with the best care.
Transfer of records form:
If you are needing to transfer your records to The Cottage Surgery from another practice? Our transfer of records form allows you to retrieve your medical records from your previous practice. Please print, complete and present this form to our reception team during your next visit to the Cottage Surgery
